  • Choctaw
  • Delaware
  • Caddo Nation of Oklahoma

You will need to provide a clear, color, full-face digital photograph. You must have an Adult Patient Physician Recommendation Form dated within 30 days of the application. If you receive Medicaid (SoonerCare) or Medicare, you will need to provide the insurance card or enrollment documentation.

How Do I Pay for a Card?

After you get approved for your recommendation you will need to apply with the state of Oklahoma. The typical fee for the state application is $100. If you have Medicaid (SoonerCare) or Medicare, you will pay a decreased fee of $20 for the state application. You must pay these fees using a Visa, MasterCard, or Discover card as either credit or debit card.

If you do not have a credit card, you may buy a pre-paid MasterCard or Visa card at a retail store. You can put the application and processing fees on this card. Then you will use the card to pay your online application fees.

What If You Need a Caregiver to Help with the Marijuana?

Some individuals are unable to administer medical marijuana to themselves. They need the help of a caregiver.

In this situation, you can get a caregiver license with documentation from the physician. You must include your Adult Patient Physician Recommendation Form with your application (don’t worry – we will help you with this!). You may also submit it at a later date by accessing your online account.

You may only have one licensed caregiver at a time. This caregiver must submit an Adult Patient Caregiver Designation Form signed by you.

What Happens Once You Finish the Application?

Now that you have submitted the forms and application, what happens next?

OMMA reviews your application. They look to see if your application is complete and meets all requirements.

You will receive an approval letter with your identification card. The card is mailed within 14 days of your application submission date.

In the case of unclear or missing information or documents, OMMA sends an email to you. They will request the needed information. You will then log into your account and make the corrections.

The submission date is the date when you submit all the information and documentation. Thus, if you have corrections, your submission date will change.

If the OMMA reviews and denies an application because all requirements aren’t met, you will receive a denial letter. This will arrive within 14 days of application submission. The letter will state the reason for the denial.

Will I Be Put on a List?

Many people fear that obtaining an MMJ will put their name on a list of marijuana users. Be assured that there is no list. The DEA does not know who has MMJs and no consolidated database exists.
