- Any condition not otherwise specified in this Subparagraph that a physician, in his medical opinion, considers debilitating to an individual patient and is qualified through his medical education and training to treat
What Do I Need To Apply For My Medical Marijuana Card?
To become a medical marijuana patient in the state of Louisiana, you will need the following documents to provide proof of residency:
- Copy of drivers license
- State ID
- Vehicle registration
- Voter’s registration
- Correspondence from another state agency with current date or a current car insurance bill
You must also submit a completed online patient intake form. If you are under 18, you will need a completed and signed form with a designated legal guardian as the primary caregiver or authorized purchaser. The caregiver must also fill out the intake form with the patients license number. The patient will receive this once their application is approved.
In Louisiana, the state does not require patients to register or receive a physical medical marijuana card. Your doctor will fax your patient certification to the dispensary of your choice.
