Who qualifies in Massachusetts?
Adults 18 and older generally need Massachusetts residency, a debilitating medical condition, and certification from a state-registered healthcare provider.
Massachusetts law names these conditions:
- Cancer
- Glaucoma
- HIV-positive status
- AIDS
- Hepatitis C
- ALS
- Crohn’s disease
- Parkinson’s disease
- Multiple sclerosis
Other debilitating conditions may qualify when an authorized clinician determines this in writing. Having a diagnosis does not guarantee certification.

Can I get my first medical cannabis evaluation online?
Yes. Current Massachusetts regulations permit an initial live, face-to-face telehealth evaluation at the patient’s request. The clinician must still complete the required assessment.
How soon can I use my certification?
Eligible newly certified patients may receive Initial Access for up to 14 days or until registration approval, whichever comes first. The usual allowance is 2.5 ounces, with clinician-authorized exceptions. Initial Access is generally limited to once in 365 days unless the Commission approves otherwise. Complete full registration promptly.
After state approval, print a temporary registration card from your portal. The physical card typically arrives within one to three weeks. This temporary card is separate from Initial Access.
Does medical certification last one year or two?
Certification duration and annual state registration are separate. Certain patients receiving SSDI or SSI, or meeting terminal-illness or permanent-disability provisions after an in-person assessment, may qualify for two-year certification. Ask your clinician which requirements apply.
Can minors, nonresidents, or caregivers participate?
Minors have a separate pathway involving two certifying physicians and a parent or guardian. Certain nonresidents receiving cancer treatment, palliative care, or end-of-life care in Massachusetts may also qualify. Contact us before booking to confirm service availability for these circumstances.
Personal caregivers must be at least 21 and complete patient designation and state caregiver registration. Review caregiver requirements.
