1. Find your state registration email
If your physician recommends medical cannabis, the physician enters your recommendation and profile into the state Patient & Caregiver Registry. Check the email address you provided at your appointment.
Ohio identifies the sender as [email protected] and the subject as ACTION NEEDED: Ohio MMCP Registry – Account Registration. Check your spam or junk folder if you do not see it.
See Ohio’s physician-to-Registry registration process.
2. Open your Registry profile and check your details
Use the link in the state’s email and follow its account-setup instructions. Review your name, date of birth, email, address and identification details carefully.
If anything is incorrect, ask your recommending physician to correct the submitted information. Do not create extra Registry accounts to work around a profile problem.
3. Complete activation
Follow the legal statements and activation instructions shown in your Registry profile. Ohio’s guidance identifies an ACTIVATE CARD button. A physician recommendation alone is not the same as an active Registry card.
Applying with veteran or indigent status?
The state may need to review your status documents before activation becomes available. Ohio says the activation button is unavailable while that special-status review is pending. Follow the state’s instructions for documentation and contact information.
Read the complete official activation instructions.
4. Download your digital card
After activation, download your Registry card and save it to your phone or print a copy. Ohio does not mail a physical Registry card.
Before visiting a licensed dispensary, check that:
- Your Registry card is active.
- Your physician recommendation is active.
- You have the valid government-issued photo ID linked to your Registry account.
Bring your digital or printed Registry card and photo ID. Cannabis purchases are separate from your physician evaluation, and dispensary staff can help you understand their first-visit requirements.

Already registered? Renew your existing account
Ohio renewal has two parts: a new physician recommendation and renewal of your Registry registration. Sign in to your existing Registry account rather than starting a duplicate account.
- Meet with a physician for a new recommendation.
- Sign in to your Ohio Registry account.
- Choose RENEW CARD when available, review the information and download the updated card.
You can submit registration renewal within 30 days before expiration. If your registration and recommendation have already expired, get the new recommendation before renewing. Follow Ohio’s current renewal instructions.
