How to Get a Medical Marijuana Card
For starters, if you want to buy and use medical marijuana in Missouri, you will also need a medical marijuana card. The Medical marijuana card’s official name is a medical marijuana (MMJ) recommendation. Your MMJ allows you to purchase medical marijuana.
You must provide a medical prescription written by a licensed healthcare provider to a Missouri licensed dispensary facility. Qualifying patients may be certified by an MD, DO, or a nurse practitioner. The professional must have a Missouri medical license and be in good standing with the state of Missouri.
The application fee for an MMJ is $25 per patient or primary caregiver. As of December 8th, 2022, patient/caregiver cultivation ID fees are reduced to $50.
Beginning May 3rd, 2023, dispensaries will be required to scan patient and caregiver cards at the time of purchase.
Requirements for a Medical Marijuana Card in Missouri?
There are several steps you must take to get an MMJ. First, you must be 18 years or older. Your application must include a “Physician Certification” and must be dated no less than 30 days.
You may submit the Physician Certification in written or electronic format. It must also include your physician’s signature. This signature means that, in their professional opinion, medical marijuana will benefit your condition.
The patient must attach their Physician Certifications to their applications; they are not generally sent from the physician’s office. You will need to download this form from the state medical marijuana website. Then, you’ll want to get it filled out by a physician and get their signature.
Physician Certification Form
Your application that will be submitted to the state of Missouri will need your “Physician Certification Form”.
On December 8th, 2022, all approved patient ID card holders will be allowed to purchases 6 ounces of marijuana per month. There are two different types of forms depending on how much medical marijuana you need. The Standard Physician Certification Form is for patients looking to get a dose of 6 ounces of marijuana per month.
Patients requiring more than 6 ounces per month will need a physician certification form that certifies them for an increased amount. The form for more than 6 ounces is called the Alternative Physician Certification Form.
This form must be completed by two different physicians. Additionally, both forms must also include the same qualifying condition. In other words, one doctor cannot say you have cancer while the other doctor says you have glaucoma. If the physicians’ recommended amounts are not the same, the patient will be given the lesser of the two.
Patients under 18 will need to accompany the Physician Certification Form in addition with the Parent/Legal Guardian Consent Form. So if you are under 18, you will need to submit two forms.
Caregivers will need to submit a Patient Authorization Form and the patient’s license number. The patient will receive this once their application has been approved.
Missouri Medical Marijuana Qualification
Missouri has a list of qualifying conditions in order to get an MMJ. Before starting the application process, verify that you have a qualifying condition diagnosis. The physician must determine if you have had prior treatment failure. In other words, this means a lack of response to treatment regimens or adverse symptoms or side effects.
To get approved for a qualifying condition, you do not need medical documentation. However, patients can provide proof of a chronic medical condition. Proof can include medical records, prescription pill bottles, office visit records, or a note from your doctor.
Some conditions included on the qualifying conditions list are:
- Hepatitis C
- Amyotrophic lateral sclerosis
- Inflammatory bowel disease
- Crohn’s Disease
- Huntington’s Disease
- Autism
- Neuropathies
- Sickle cell anemia
- Agitation associated with Alzheimer’s Disease
- Cachexia
- Wasting syndrome
Chronic medical conditions causing severe, ongoing pain or muscle spasms may include:
- Multiple Sclerosis
- Seizures
- Parkinson’s Disease
- Tourette’s Syndrome
Psychiatric disorders can consist of Post-traumatic stress disorder (PTSD). The diagnosis of all psychiatric conditions must come from a state licensed psychiatrist.

Information You Will Need for Your Application
You must submit the application using the Missouri state online registry. Before you submit, you’ll need to register and make an account.
You must provide identifying information to get certified. This includes your full name, date of birth, social security number, and your residential and mailing address. According to the new laws, you do not need proof of Missouri residency to apply.
As mentioned earlier, you need a signed Physician Certification Form. The physician’s signature should be within 30 days of application submission. Be sure the physician holds a Missouri license that is active and in good standing.
You must additionally provide a legible copy of your photo identification issued by a state or government agency.
Also, you must provide a clear, color, digital photo of the applicant’s face. The photo must be full face, from the shoulders up. Additionally, you may not use filters, sunglasses, or hats in your photo.

Submitted a patient, caregiver, or cultivation application on December 8th, 2022 or later? Then it will be valid for 3 years.
If you were approved before December 8th, 2022, your card will be valid for only one year.
Patients and caregivers can apply to renew their card up to 60 days before the expiration date.
You Can Cultivate Medical Marijuana
On February 6th, 2023, Missouri’s medical program will start accepting applications for personal cultivation. If you do not qualify for a medical marijuana card, you will be allowed to cultivate plants for personal use. You can apply for personal cultivation as long as you are at least 21 years old.
However, if you do have your medical marijuana card already, you will be able to grow without submitting an application.
The qualifying patient or caregiver may only have one cultivation license.
You must adhere to several state requirements. You may have the plants indoors or outdoors. However, they must always be in a safe, locked, and fully enclosed space enclosed and locked. Individuals may only grow six flowering marijuana plants, but you can keep another six plants in the vegetative state.
