Oklahoma adult licenses last up to two years, not one. To renew, Broken Arrow patients need a new signed Physician Recommendation Form and must pay OMMA’s fee again ($104.30, or $22.50 reduced). PrestoDoctor renewals are $79, and you pay only if approved.

Fees
OMMA charges $100 plus a $4.30 card processing fee ($104.30), or $20 plus $2.50 ($22.50) with proof of Medicaid (SoonerSelect), Medicare, or 100% disabled veteran status. The fee is nonrefundable.
